What is Anticoagulation Management Service?
A clinical, pharmacist-run service for Grand Rapids and West Michigan patients to monitor and adjust warfarin (Coumadin) for the period of time patients need anticoagulation therapy.

How to Enroll
A referral from your doctor is required for participation in the clinic. After receiving a referral authorization, you will be contacted by phone to schedule your initial visit.
